Nested loops: grids, tables and pairs

Put one loop inside another to work through rows and columns, build tables, list every pair, and understand how the work grows.

• Trace a loop inside another loop and predict how many times the inner body runs
• Print grids and tables row by row
• List every pair of items exactly once by starting the inner loop after the outer value
• Reset per-row accumulators in the right place
